Grounded in a harm reduction and cultural competence philosophy, the Substance Use Care Stream supports innovations and best practices in both community and hospital-based care. The Substance Use Care Stream focuses on building a connected and equitable system where people can easily access the services and supports they need across the substance use continuum, from harm reduction and treatment. Our goal is for community services to work together to create clear, simple, and seamless care pathways, so people receive consistent support from first contact through ongoing care.
